An authentication plugin to ues REMOTE_USER is in 
http://trac.roundcube.net/ticket/1486689

To do anything useful with that, I implemented support for SASL proxy 
authentication for

- IMAP  (http://trac.roundcube.net/ticket/1486690)
- SIEVE (http://trac.roundcube.net/ticket/1486691)
- LDAP  (http://trac.roundcube.net/ticket/1486692)
- SMTP  (http://trac.roundcube.net/ticket/1486693)

These latter four patches may be useful on their own though.

They have been tested on a system running Apache2/mod-kerberos + 
Cyrus-IMAP + Postfix + OpenLDAP.
