You need to create three database tables:
(*) events
(*) events_cache
(*) events_caldav

The scripts are located in the plugin's SQL folder.

To connect to a CalDAV you just need to toogle the
Provider select box in calendar settings. When saved
you'll see input fields to insert the access data.
