For example - what to do with open bugs relating
to old versions? What to do with bugs that have no version, or no
milestone?

Concerning trac bugs. There are still al lot of
bugs without a version, or without a milestone. This is confusing to users and
even more confusing to the development team as these bugs fall between
releases.
