We are seeing something similar after upgrading to 0.9.1, and it seems to be related to:

.../program/resources/blank.pdf .../program/resources/blank.tif

Only some of our testers are seeing this behavior, though, with some versions of firefox.

is this related to http://trac.roundcube.net/ticket/1488972 perhaps?
