Firefox 21 Linux, no Plugins, RC 0.9.1 about:config -> pdfjs.disabled -> true

I disabled all pdf-related plugins, including pdfjs and the request for blank.pdf is send, but still no warning. Maybe I should disable all plugins, even those not related to pdf?

it shouldn't, this capability check is skin-independent.
It also happen to me on Firefox 17.0.6 ESR and firefox nightly build. It is OK on Chromium.
