One of the most annoying things I've found so far in roundcube is that if you have a threaded message listing and have expanded a thread, that this when returning to the message listing from viewing a full message in this thread. It is obvious enough why it works this way but it would be fantastic if the message listing state (and position) was preserved so users don't have to search around in the message listing to to pickup where they left off.

It is true. However, you can use preview pane or "open in new window" feature. So, it definitely is not a priority.

True on both counts, but the same issue exists if you reply to a message in a thread (and aren't composing in a new window.) Not every user is savy enough to deal with or learn how to change their behavior to work around it.

They don't need to change their behaviour. But just their config in two places.
BTW: I wasn't familiar with the term "savy", googled it, and found in http://www.urbandictionary.com/define.php?term=savy:
"savy- a word used by those whom of which do not know how to spell savvy."
;-) I wasn't familiar wirh "savvy" as well, but at least I were able to find a meaningful translation with this correct spelling. No offense.

Would it be a very big deal to offer the facility to create such intro lines (including DOW and TZ) with Roundcube as well, as Mutt apparently does?
For sure not one of the most important issues, but one of those I always wanted to ask for ever since.
Of course, a selection of the various options in a drop-down list would be perfect.
